RESUMO:
O envelhecimento populacional no Brasil e no mundo, assim como o aumento das
internações de idosos em serviços hospitalares e, especificamente, em Unidades de
Terapia Intensiva, tem mostrado a importância de identificar os fatores associados
ao desfecho clínico das internações, nessa faixa etária, em tais unidades. Nesse
sentido, esta investigação objetivou analisar os fatores associados ao desfecho
clínico de idosos internados em Unidades de Terapia Intensiva de hospitais de
referência. Estudo observacional, transversal, retrospectivo, desenvolvido em duas
instituições hospitalares uma pública e outra filantrópica do município de Teresina –
Piauí, que incluiu indivíduos com 60 anos ou mais de idade, os quais foram
internados nas Unidades de Terapia Intensiva desses serviços no ano de 2014. A
coleta de dados foi realizada entre junho e julho de 2016, utilizando-se formulário
construído pelos pesquisadores e validado por três juízes. Para a análise dos dados
utilizou-se o programa Statistical Package for the Social Sciences 20.0. Para as
variáveis qualitativas usou-se a distribuição de frequências absolutas (n) e relativas
(%) e nas quantitativas utilizaram medidas de tendência central (média e mediana) e
dispersão (desvio padrão); os testes qui-quadrado de Pearson e Anova (One-way
Anova), com nível de significância de 5 %, foram adotados para realizar a estatística
inferencial. Participaram do estudo 238 idosos, cuja média de idade foi de 74,2 anos,
sendo 51,6% homens, casados (55,8%), com ensino fundamental (46,2%), de cor
parda (33,1%) e aposentados (63,8%). Verificou-se diferença importante no perfil
clínico dos idosos internados nos dois serviços, mas no geral 49,5% eram
procedentes de outras unidades do próprio hospital. Observou-se 16% de
reinternação, com duração média de 7,2 dias, as quais apresentaram como
desfecho o óbito em 68,5% dos casos. O pós-operatório imediato foi responsável por
31,9% das internações. Dentre os fatores avaliados, a doença cerebrovascular foi a
doença crônica não transmissível mais frequente (66%); a escala de coma de
Glasgow teve avaliação grave na admissão, observou-se uso de ventilação
mecânica e droga vasoativa durante a internação, o perfil glicêmico apontou
hiperglicemia, sendo que todos os fatores investigados tiveram significância
estatística com o desfecho óbito. Conclui-se que o envelhecimento saudável, com
redução de fatores evitáveis como as doenças crônicas não transmissíveis e o
controle das taxas fisiológicas podem levar a um melhor prognóstico caso o idoso
venha a necessitar de cuidados intensivos...................ABSTRACT: Population aging in Brazil and in the world, as well as the increase in the

factors evaluated, cerebrovascular disease was the most frequent non-transmissible

hospitalization, the glycemic profile showed hyperglycemia, and all the factors
investigated had statistical significance with the death outcome. It is concluded that
healthy aging, with reduction of avoidable factors such as chronic non-communicable
diseases and the control of physiological rates can lead to a better prognosis if the
elderly are in need of intensive care. |
