Abstract:
RESUMO: Introdução: O estilo de vida determina a heterogeneidade do envelhecimento. E, as práticas
educativas em saúde, associadas aos recursos motivacionais, são fundamentais no processo de
adaptação às mudanças psicofisiológicas e à adoção e manutenção de comportamentos
saudáveis em idosos. Objetivo: Avaliar a eficácia de intervenção educativa e motivacional na
promoção da saúde de idosos em risco de fragilidade. Método: Estudo multimétodos, composto
por pesquisa metodológica e Ensaio Clínico Randomizado (ECR), na Atenção Primária à
Saúde, de Teresina, Piauí, Brasil. No estudo metodológico, ocorreu construção do vídeo
educativo “Prosa de um recém idoso” sobre fragilidade em idosos. O vídeo foi validado por 22
juízes especialistas e avaliado por 22 idosos. Os dados foram analisados pelo Índice de
Validação de Conteúdo (IVC) e teste binomial. O ECR teve amostra de 108 idosos, divididos
em dois grupos distintos, distribuídos randomicamente. O Grupo Intervenção (GI) (n=55)
participou de intervenção educativa e motivacional, subsidiada pelo vídeo educativo e por
entrevista motivacional breve; e o Grupo Controle (GC) (n=53) recebeu orientações verbais de
rotina nos serviços de atenção primária à saúde. Os desfechos avaliados foram a capacidade de
autocuidado, autopercepção de saúde, senso de autoeficácia e prontidão para mudança de
comportamento, por meio de questionário semiestruturado e pela Escala para Avaliar a
Capacidade de Autocuidado (EACAC). A coleta de dados ocorreu no domicílio e nas unidades
básicas de saúde, em dois momentos: pré-teste e aplicação da intervenção e pós-teste, após 30
dias, tendo como referencial teórico-metodológico o Modelo de Promoção da Saúde de Nola
Pender. Os testes estatísticos foram realizados com nível de significância de 5% e intervalo de
confiança de 95%. Utilizaram-se dos softwares R versão 3.5.1 e o Statistical Package for the
Social Sciences, versão 21.0. O estudo seguiu a regulamentação do Conselho Nacional de
Saúde, conforme a Resolução 466/12, e os resultados foram cadastrados no Registro Brasileiro
de Ensaios Clínico. Resultados: A versão final do vídeo é composta por 11 minutos e 25
segundos e aborda as recomendações para idosos em risco de fragilização e hábitos promotores
da saúde. Verificou-se concordância superior a 90% em todos os itens avaliados pelos juízes e
pelo público-alvo. No ECR, os grupos se mostraram homogêneos na linha de base quanto às
variáveis sociodemográficas, econômicas e clínicas (p>0,05). Na comparação intergrupo,
verificaram-se efetividade clínica quanto à capacidade de autocuidado, autopercepção de saúde
e prontidão para mudança de comportamento. Na análise intragrupo, o GI apresentou aumento
da capacidade de autocuidado (p<0,001) e do senso de autoeficácia (p<0,001), melhoria da
autopercepção de saúde (p=0,001) e evolução para fase de ação motivacional (p=0,001), com
diferenças estatisticamente significativas. E o GC apresentou melhoria significativa apenas no
senso de autoeficácia (p<0,001). Conclusão: A intervenção educativa e motivacional para
promoção da saúde de idosos em risco de fragilidade apresentou eficácia clínica superior às
orientações verbais de rotina, ocasionando diferença na capacidade de autocuidado,
autopercepção de saúde, prontidão para mudança de comportamento e senso de autoeficácia.
ABSTRACT: Introduction: Lifestyle determines the heterogeneity of aging. And educational health
practices associated with motivational resources are fundamental in the process of adapting to
psychophysiological changes and the adoption and maintenance of healthy behaviors in the
elderly. Objective: To evaluate the effectiveness of an educational and motivational
intervention in promoting the health of elderly people at risk of frailty. Method: Multi-method
study, consisting of methodological research and Randomized Clinical Trial (RCT) in Primary
Health Care in Teresina, Piauí, Brazil. In the methodological study, the construction of an
educational video “Prose of a newborn” about frailty in the elderly. The video was validated by
22 experts’ judges and evaluated by 22 elderly people. The data were analyzed using the
Content Validation Index (CVI) and binomial test. The RCT had a sample of 108 elderly people,
divided into two distinct groups, randomly distributed. The Intervention Group (IG) (n = 55)
participated in an educational and motivational intervention, subsidized by the educational
video and a brief motivational interview; and Control Group (CG) (n = 53) received routine
verbal guidance in primary health care services. The outcomes assessed were the ability to selfcare,
self-perceived health, sense of self-efficacy and readiness for behavior change, through a
semi-structured questionnaire and the Scale for Assessing Self-Care Capacity (EACAC). Data
collection took place at home and in basic health units, in two moments: pre-test and application
of the intervention and post-test, after 30 days, using the Nola Pender Health Promotion Model
as a theoretical and methodological framework. Statistical tests were performed with a 5%
significance level and a 95% confidence interval. The software R version 3.5.1 and the
Statistical Package for the Social Sciences version 21.0 were used. The study followed the
regulation of the National Health Council, through Resolution 466/12 and the results registered
in the Brazilian Registry of Clinical Trials. Results: The final version of the video consists of
11 minutes and 25 seconds and addresses the recommendations for elderly people at risk of
fragility and health-promoting habits. An agreement greater than 90% was verified in all items
evaluated by the judges and by the target audience. In the RCT, the groups were homogeneous
at the baseline regarding socio-demographic, economic and clinical variables (p> 0.05). In the
intergroup comparison, clinical effectiveness is verified in terms of the capacity for self-care,
self-perception of health and readiness for behavior change. In the intragroup analysis, the IG
showed an increase in the capacity for self-care (p <0.001) and a sense of self-efficacy (p
<0.001), improvement in self-perceived health (p = 0.001) and evolution to the motivational
action phase (p = 0.001) , with statistically significant diferences. And the CG showed
significant improvement only in the sense of self-efficacy (p <0.001). Conclusion: The
educational and motivational intervention to promote the health of elderly people at risk of
frailty showed clinical efficacy superior to routine verbal guidelines, causing a difference in the
capacity for self-care, self-perceived health, readiness to change behavior and sense of selfefficacy.
