RESUMO:O alendronato (ALD) é um bifosfonato amplamente utilizado no tratamento da osteoporose, contudo apresenta algumas reações adversas, dentre eles o surgimento da úlcera gástrica. Em contrapartida, a metformina (MET) apresenta diversos efeitos benéficos ao organismo, além de ser utilizada na clínica para controle de níveis glicêmicos em pacientes diabéticos, pode também favorecer a cicatrização de feridas epiteliais, por exemplo. O objetivo do estudo foi investigar o efeito gastroprotetor da MET em lesões gástricas induzidas por ALD em ratas normoglicêmicas e hiperglicêmicas. Para isso ratas foram divididas em controle negativo (solução fisiológica, SF 0,9%, v.o); controle positivo (SF 0,9% + ALD 50mg/Kg, v.o), três grupos com MET (MET 10, 30 ou 100mg + ALD 50mg/Kg, v.o), um grupo com inibidor de AMPK (Comp C 1,2mg/Kg + MET 100mg/Kg + ALD 50mg/Kg, v.o), controle negativo diabético (SF 0,9%, v.o), controle positivo diabético (SF 0,9% + ALD 50mg/Kg, v.o) e diabético na melhor dose (MET 100mg/Kg + ALD 50mg/Kg, v.o). O diabetes foi induzido com estreptozotocina (STZ), 130mg/Kg, intraperitoneal, dose única. Os pré-tratamentos foram realizados diariamente durante sete dias e a lesão gástrica foi induzida com ALD 50mg/Kg diariamente, durante quatro dias. Observou-se que a dose de MET 100mg/Kg apresentou melhores resultados que as demais testadas, sendo então selecionada para a execução dos protocolos posteriores. A MET desempenhou efeito gastroprotetor significativo em ensaios de lesão gástrica, tanto em análise macroscópica como microscopia de luz e de força atômica. A MET foi capaz de suprimir fatores ligados à inflamação, como a expressão de citocinas pró-inflamatórias (TNF-α, IL-1β e IL-6) e diminuição dos níveis de MPO, além da redução dos níveis de MDA. Também foi observado manutenção dos níveis basais de colágeno na mucosa gástrica e seu potencial de estímulo à secreção de muco gástrico. Pode-se observar comprometimento da ação gastroprotetora da MET no grupo com a presença de inibidor de AMPK, sugerindo a participação dessa via na sua ação gastroprotetora. Conclui-se que a MET apresenta efeitos tanto de diminuição da resposta inflamatória como proteção mecânica, com o estímulo à produção de muco, o que sugere sua potencial gastroproteção contra lesão por ALD.
ABSTRACT:Alendronate (ALD) is a bisphosphonate widely used in the treatment of osteoporosis, but it has some adverse reactions, among them the appearance of gastric ulcer. In contrast, metformin (MET) has several beneficial effects on the organism, besides being used clinically to control glycemic levels in diabetic patients, it may also favor the healing of epithelial wounds, for example. The aim of the study was to investigate the gastroprotective effect of MET on gastric lesions induced by ALD in normoglycemic and hyperglycemic rats. For this, rats were divided into negative control (physiological solution, SF 0.9%, p.o); positive control (SF 0.9% + ALD 50mg / kg, p.o), three groups with MET (MET 10, 30 or 100mg + ALD 50mg / kg, p.o), one group with AMPK inhibitor (Comp C 1.2mg / kg + MET 100mg / kg + ALD 50mg / kg, p.o), diabetic negative control (SF 0.9%, p.o), diabetic positive control (SF 0.9% + ALD 50mg / kg, p.o) and diabetic at the best dose (MET 100mg / kg + ALD 50mg / kg, p.o). Diabetes was induced with streptozotocin (STZ), 130mg / kg, intraperitoneal, single dose. The pre-treatments were performed daily for seven days and the gastric lesion was induced with ALD 50mg / kg daily for four days. It was observed that the dose of MET 100mg / kg presented better results than the others tested, being then selected for the execution of the subsequent protocols. MET had a significant gastroprotective effect in gastriclesion assays, both in macroscopic analysis and in light and atomic force microscopy. MET was able to suppress inflammatory factors such as the expression of proinflammatory cytokines (TNF-α, IL-1β and IL-6) and decreased MPO levels, in addition to the reduction of MDA levels. Maintenance of basal collagen levels in the gastric mucosa and its potential to stimulate the secretion of gastric mucus were also observed. It can be observed compromised gastroprotective action of MET in the group with the presence of AMPK inhibitor, suggesting the participation of this pathway in its gastroprotective action. It is concluded that MET has effects both of decreased inflammatory response and mechanical protection, with the stimulation of mucus production, which suggests its potential gastroprotection against AML lesion. |
