Controle que transforma: o impacto da atuação do Tribunal de Contas em políticas públicas municipais de saúde no Piauí

Abstract

RESUMO: Esta dissertação trata do controle externo realizado pelo Tribunal de Contas do Estado do Piauí (TCE-PI) através do Plano Anual de Controle Externo (PACEX), bem como das implicações disso em relação às políticas municipais de saúde. Em linhas gerais, objetiva-se diagnosticar os impactos gerados pela atuação do TCE-PI a partir da atuação estruturada pelo PACEX nas políticas públicas, públicas de saúde dos municípios piauienses. Este trabalho visa explicar, por meio de uma pesquisa bibliográfica e documental, a relação entre o controle externo e a avaliação das políticas de saúde realizada pelo TCE-PI. Para tanto, foram analisados relatórios do TCE-PI, parâmetros técnicos do Instituto Rui Barbosa, dados de saúde municipal disponibilizados pelo Ministério da Saúde e outros documentos relativos à prestações de contas dos municípios. Dois instrumentos serão a base desta análise: o Plano Anual de Controle Externo do TCE-PI, que detalha o planejamento das ações de fiscalização, e a metodologia do Índice de Efetividade da Gestão Municipal (IEGM). A apreciação foca no i-Saúde, índice que mede o resultado das ações de gestão pública municipal em saúde, com ênfase na Atenção Básica, no Programa Saúde da Família, no Conselho Municipal de Saúde, na assiduidade médica, no atendimento a doenças como tuberculose, na prevenção de dengue, no controle de insumos e nas campanhas de vacinação. O material encontra-se organizado em três capítulos. No primeiro, é apresentado um referencial teórico sobre o controle externo exercido pelo Tribunal de Contas no Brasil, expondo pesquisas acadêmicas sobre o tema e sobre a avaliação de políticas de saúde. Além disso, é apresentada a estrutura conceitual do PACEX do TCE-PI. No segundo capítulo, são discutidos os parâmetros de análise dos impactos do plano de controle na avaliação das políticas municipais de saúde e os resultados verificados. E, por fim, no terceiro capítulo, é avaliada a atuação do PACEX e do TCE-PI, destacando insuficiências, oportunidades, ameaças e sucessos. ABSTRACT: The external control carried out by the Court of Auditors of the State of Piauí through the Annual External Control Plan (PACEX) is discussed, as well as the implications of this in relation to municipal health policies. In general terms, the objective is to diagnose the impacts generated by the actions of the Court of Auditors of the State of Piauí based on the actions structured by the Annual Control Plan – PACEX on public health policies in the municipalities of Piauí. This work aims to explain, through bibliographical and documentary research, the relationship between external control and the evaluation of health policies carried out by the Court of Auditors of the State of Piauí. To this end, TCE-PI reports, technical parameters from the Rui Barbosa Institute, municipal health data made available by the Ministry of Health and other documents relating to municipalities’ accountability were analyzed. Two instruments will be the basis of this analysis: the TCE-PI Annual External Control Plan, which details the planning of inspection actions, and the methodology of the Municipal Management Effectiveness Index (IEGM). The assessment focuses on I-SAÚDE, an index that measures the results of municipal public health management actions, with an emphasis on Primary Care, the Family Health Program, the Municipal Health Council, medical attendance, care for diseases such as tuberculosis, in the prevention of dengue fever, in the control of inputs and in vaccination campaigns. The material is divided into three chapters. In the first, a theoretical framework is presented on the external control exercised by the Court of Auditors in Brazil, exposing academic research on the topic and on the evaluation of health policies. Furthermore, the conceptual structure of TCE-PI’s PACEX is presented. In the second chapter, the parameters for analyzing the impacts of the control plan on the evaluation of municipal health policies and the results verified are discussed. And finally, in the third chapter, the performance of PACEX and TCE-PI is evaluated, highlighting shortcomings, opportunities, threats and successes.
