NOS BATUQUES DOS QUINTAIS: As interfaces do cuidado em saúde entre religiões de terreiros e equipes de saúde da família

Abstract

RESUMO: A criação do Sistema Único de Saúde (SUS) levou a mudanças de perspectivas que resultaram em críticas ao modelo hospitalocêntrico e maior enfoque na Atenção Primária à Saúde (APS). Na realidade brasileira, esse nível é operacionalizado principalmente pela Estratégia de Saúde da Família (ESF). Às ESF’s é incumbido que as mesmas interajam com os espaços sociais da comunidade e promovam cuidados em saúde através de relações democráticas, equânimes e horizontais. De modo geral, essas interações da ESF com as práticas de cuidado em saúde promovidas pelos povos terreiros mostram-se necessárias. O objetivo desse estudo consistiu em investigar os processos de interações entre religiões de terreiros e ESF’s nas ofertas de cuidado em saúde aos territórios em que estão inseridos e aos adeptos/consulentes das religiões afro-brasileiras. Para isso, utilizamos o referencial da Análise Institucional “no Papel”. Os sujeitos foram divididos em três grupos: no primeiro tivemos três líderes de centros religiosos afro-brasileiros; no segundo, praticantes/consulentes desses estabelecimentos religiosos; e no terceiro, os profissionais das três ESF que cobriam os respectivos terreiros investigados. Como instrumentos, utilizamos entrevistas semi-estruturadas com os líderes religiosos; observação participante das cerimônias espirituais; conversas informais com os consulentes; e uma roda de conversa com cada ESF. Promover saúde nesses espaços expressa perspectivas de cuidado em saúde que se contrapõem às racionalidades biomédicas, positivistas e cartesianas, fazendo referência a terapêuticas como: uso de plantas com finalidades terapêuticas; recebimento de rezas e passes; consultas com cartomantes; atendimentos com as divindades e desenvolvimento de projetos sociais e comunitários. As ESF’s terminam funcionando num viés curativo-preventivista, o que desemboca na premissa de que seriam os profissionais da saúde os detentores “oficiais” das práticas de cuidado. Dentre as possibilidades de dialogias, conseguimos identificar as seguintes: ausência de atividades conjuntas; reconhecimento mútuo sobre a eficácia e a importância do cuidado em saúde promovido tanto pelos terreiros como pelos serviços de saúde; desenvolvimento de ações conjuntas por iniciativa das ESF’s; e atividades promovidas pelos povos de terreiros em decorrência das suas articulações políticas, sendo esse último caminho deveras enriquecedor para se pensar as interfaces serviços-terreiros. ABSTRACT: The creation of the Brazilian Unified Health Care System (SUS) led to the defense of health perspectives contrary to the hospital-centered model and a greater focus on Primary Health Care (APS). In Brazilian reality, this level of complexity is mainly implemented by the Family Health Strategy (ESF). These teams are responsible for interacting with the social spaces of the community, as well as promoting health care through democratic, equitable and horizontal relationships. In general, these interactions of the ESF with the health care practices promoted by the terreiros are necessary. This study aimed to investigate the processes of interaction between religions of terreiros and ESF’s in health care offerings to the territories in which they are inserted and to the adepts /consulters of afro-brazilian religions. For this, we use the Institutional Analysis "in the Paper". The participants were divided into three groups: in the first, we had three leaders from Afro-Brazilian religious centers; in the second, practitioners / consultants of these religious establishments; and in the third, the professionals of the three ESF that were located in the communities of the respective terreiros investigated. As instruments, we use semi-structured interviews with religious leaders; participant observation of spiritual ceremonies; informal conversations with the consultants; and a conversation wheel with each ESF. The promotion of health in these spaces expresses perspectives of health care that are opposed to the biomedical, positivist and Cartesian rationalities, referring to therapeutics such as: use of plants for therapeutic purposes; receiving prayers and passes; consultations with fortune tellers; services with the deities and development of social and community projects. The ESF’s, for the most part, work from a curative and preventive perspective, leading to understandings that health professionals are the "official" custodians of care practices. In this research, we were able to identify the following possibilities of ESF-terreiros interfaces: absence of joint activities; mutual recognition on the effectiveness and importance of health care promoted by both terreiros and health services; development of joint actions at the initiative of the ESF; and activities promoted by the people of the terreiros as a result of their political articulations, the latter being a very important way to think about the interfaces between the terreiros.
